Cambridge Public Schools offers a simple, secure solution to completing forms related to your child's education. The forms are easy to complete and all data is protected and secure.
In order to get started, you will need your child Student ID#. Here are some tips on finding this number:

- If you still need help, contact your school-based Family Liaison (or OSS Case Manager) via email to request a call-back with the number.
If you have more than one child needing signed documents, please log out and then log back in with each child's information.
All forms should be completed by parents / guardians / or caregivers unless otherwise noted.
