CPS Community Partner Portal

The Cambridge Public Schools ("CPS") community partner identified below as well as its officers, directors, employees, representatives and agents (hereinafter “Community Partner”) hereby acknowledge and agree that it will be given a user name and password in order to access the CPS Community Partners Portal in order to virtually connect and interact with groups of CPS students who are enrolled in the Community Partner’s program. The Community Partner further acknowledges and understands that the CPS Community Partners Portal will provide it and its applicable staff with access to the groups of students that are enrolled in its program through CPS-supported technological tools and platforms, including its CPS Google-suite platform that includes Google Classroom (where programming content can be posted to be viewed and interacted with by your students) and Google Meet (that can be used by educators to virtually connect and interact with groups of students).

The Community Partner further acknowledges that it will be provided with access to selected student record data if the parents/guardians/caregivers of a child participating in their program has authorized the release of such student record data to the Community Partner. The Community Partner further agrees that it and its staff will comply with all rules and regulations regarding or relating to the release student record data and shall instruct its staff not to publish, disseminate or otherwise release any confidential information without first obtaining review and written approval of the affected individuals.

The Community Partner further represents and warrants that it will not use the CPS Community Partners Portal for any other purpose or for contacting, soliciting, selling, marketing or otherwise promoting its program or services to other CPS students and their families.

In connection with being given access to the CPS Community Partners Portal the undersigned Community Partner and the undersigned staff of the Community Partner represent and warrant, jointly and severally, that all passwords to the CPS Community Partners Portal that are received will be not be distributed, published or otherwise disseminated and will be protected as confidential. Additionally, the undersigned Community Partner and the undersigned staff of the Community Partner represent and warrant that they have read, understand and will abide by Cambridge Public Schools’ policies and procedures in connection with the use of the CPS Community Partners Portal, including without limitation, the Cambridge Public Schools’ Acceptable Use Policy, Nondiscrimination Policy and Prohibition Against Sexual Harassment, Criminal Offender Record Information Policy, and Child Abuse and Neglect Administrative Guidelines.

I hereby certify that I have read and understood this Acknowledgment of Cambridge Public School Policies, understand its terms. I sign it voluntarily and with full knowledge of its significance.
